Physical Therapist

Full Time

We are seeking a dedicated Physical Therapist to join our interdisciplinary team, providing restorative and rehabilitative therapy services in Stockton, CA. This role focuses on care planning, coordination of therapy services, and helping participants maximize independence, mobility, and safety.


The position is primarily center-based, with occasional home visits (no more than two days per week, primarily for evaluations). Therapists typically see one patient per hour, allowing time for high-quality, patient-centered care.


Compensation & Benefits:

  • Salary: $99,300 – $135,000 based on experience
  • Additional 5% geographic differential on top of base salary
  • Mileage reimbursement for home visits at the federal rate

Key Responsibilities:

  • Deliver restorative and rehabilitative physical therapy services to older adult participants.
  • Collaborate with the Interdisciplinary Team to develop and implement individualized care plans.
  • Conduct functional assessments and develop treatment programs that promote mobility, independence, and fall prevention.
  • Provide therapy services primarily in the center with occasional home visits for evaluations.
  • Monitor participant progress and maintain accurate clinical documentation.

Requirements:

  • Bachelor’s degree in Physical Therapy required; Master’s or Doctorate preferred
  • Valid California Physical Therapy License or eligibility
  • Two years of clinical experience with a frail or elderly population preferred
  • Experience in outpatient, acute care, inpatient rehab, or skilled nursing settings preferred
  • New graduates may be considered if clinical rotations included older adult care
  • Current CPR certification preferred
