Temporary Physical Therapist

Full Time

We are seeking a licensed Physical Therapist (PT) to join a dedicated interdisciplinary team within a PACE (Program of All-Inclusive Care for the Geriatrics) center located in Seneca, New York. This full-time role involves delivering compassionate, evidence-based care to a medically complex, geriatric population across both center-based and community settings.

 

Compensation: $57 - $66.50 per hour

 

Position Overview:

 

  • Schedule: Monday–Friday, 9:00 AM – 5:00 PM
  • Setting: PACE site, participant homes, and skilled nursing facilities
  • Population: Frail elderly and chronically ill adults
  • Coverage to begin in September 

Responsibilities:

 

  • Develop and administer individualized physical therapy treatment plans for medically referred participants
  • Perform patient assessments, reassessments, and discharges in accordance with departmental and regulatory standards
  • Deliver therapy services at the center or in participants' homes and residential facilities
  • Collaborate with an interdisciplinary team to create comprehensive care plans
  • Accurately document participant information including assessments, progress notes, care plans, and outcomes
  • Participate in team care planning meetings and performance improvement initiatives
  • Provide clinical supervision and instruction to PTA students and interns
  • Order appropriate equipment for discharge planning and ensure timely coordination with care teams
  • Uphold safety, cleanliness, and compliance with all federal, state, and organizational guidelines 

Qualifications:

 

  • Education: Graduate of an APTA-accredited Physical Therapy program or equivalent
  • License: Active New York State Physical Therapist license – Required
  • Experience:
    • Minimum 1 year of experience as a Physical Therapist – Required
    • Prior experience working with frail or elderly populations – Preferred
    • If less than 1 year of experience with geriatrics, training will be provided upon hire
    • Strong assessment and treatment planning abilities
    • Compassionate, team-oriented approach
    • Ability to document clearly and accurately using EMR systems
    • Reliable transportation for field visits
