We are seeking two compassionate and skilled
Community Health Nurses (CHNs) for a locum tenens assignment in a PACE (Program
of All-Inclusive Care for the Elderly) program in Northeast Bronx, NY. This is
a community-based role focusing on delivering skilled nursing care to
PACE-enrolled geriatric patients in their homes and care settings. Each nurse
will manage 4–5 home visits per day and collaborate with an interdisciplinary
team to ensure high-quality, coordinated care.
Compensation: $50-63 Per Hour
Schedules Available:
- Nurse #1: Tuesday–Saturday, 8:30 AM – 5:00 PM
(with 1-hour lunch)
- Nurse #2: Sunday–Thursday, 8:30 AM – 5:00 PM
(with 1-hour lunch)
Note: Home care experience is required.
Assignment Settings:

- PACE centers
- Hospitals
- Skilled nursing facilities
Key Responsibilities:
- Provide skilled nursing care using the full
nursing process in alignment with the PACE model
- Conduct in-home clinical assessments and
coordinate care with interdisciplinary team
- Complete accurate documentation including
Uniform Assessment System (UAS) and diagnosis coding
- Develop, implement, and revise
individualized care plans
- Participate in IDT (Interdisciplinary Team)
meetings to align care planning
- Coordinate transitions across care settings
(e.g., SNF, hospital, home)
- Educate patients and caregivers to support
safety, independence, and optimal health outcomes
- Ensure compliance with state and federal
regulations governing PACE services
- Demonstrate cultural sensitivity and patient-centered communication
Qualifications:
- Degree from an NLN-accredited nursing school
(BSN preferred)
- Active New York State RN license
- Minimum 1 year of home care (CHHA),
med-surg, or critical care experience
- Experience with elderly or frail patients – Preferred
(PACE training available)
- Valid driver’s license, personal vehicle,
and auto insurance
- Must meet NYS physical/mental health clearance standards
